Project Compassion’s Advance Care Planning Initiative provides a range of resources for end of life planning:

·        printed and online resources on Advance Care Planning

·        public forums and workshops

·        facilitator training to educate and engage community leaders and volunteers

·        trained facilitators to work with individuals and families making decisions    

·        Passing on Thoughtfully Project Compassion’s workbook featured in the NY Times

NC Secretary of State Advance Health Care Directive Registry:  www.sosnc.com

This secure state-sponsored registry allows you to download NC documents for free.  You may also register your Advance Directives online to access anywhere that the internet is available.  Your documents are protected by a pass code.  There is a fee of $10 per document for the registry.  

Caring Connections:  www.caringinfo.org

This initiative of the National Hospice and Palliative Care Organization maintains Advance Directive forms and instructions for every state free of charge as well as other information. 

Project Grace:  www.projectgrace.org

This Florida organization offers information on Advance Directives with a particular specialty in CPR.  The Advance Directive document is instructive but not to be used in place of the NC forms.   

The Carolinas Center for Hospice and End of Life Care:  www.carolinasendoflifecare.org

The Carolinas Center offers an Advance Care Planning guide called “Isn’t It Time We Talk?”  This guide is available directly through the Center or through Project Compassion. 

Five Wishes:  www.agingwithdignity.org

This national organization offers the “Five Wishes” Advance Care Planning document.  This document uses clear and understandable language to help explore end-of-life options and document wishes.  The Five Wishes can function as a legal advance directive in NC and most US states.  Check the list inside the cover of the document for a complete list of states.

Palliative and End of Life Pearls by Dr. Ira Byock, MD - a new book on palliative and end of life care presents 75 case presentations of clinical interest related to end of life and palliative care for both hospitalized patients and patients receiving home care. The patient vignettes highlight considerations of pain and symptom management at the end of life, ethical issues related to life-supportive care and approaches to assisting patients and families with the difficulties that surround death and dying.

ABA's Review Of End Of Life Legislation In the US - The American Bar Association Commission on Legal Problems for the Elderly released its annual report examining end of life care legislation enacted on the state level during the 2001 calendar year.  The report gives a detailed description of newly enacted laws covering the areas of surrogate decision-making, do-not-resuscitate orders and palliative care.
